Dreamworks Questionnaire
The following questions will help me make a suitable match between a Dreamworks puppy and you. If you have additional information that you would like to share, or need more room to respond to the questions below, please use additional paper. The more information I have, the more data I have to work with when making a match, so let me know anything that you feel is important. The questionnaire can be returned to me by clicking the e-mail button above. Or, if you prefer, the form can be printed out and mailed to me. Just call for the address. 4. Have you ever owned a Border Collie or other dogs before? What happened to them? Have you ever relinquished a dog to an animal shelter or rescue?
5. Is everyone in your family in favor of getting a pup at this time?
6. Have you, or anyone in your household, ever had a negative experience with a dog?
